Эпидемическая циркуляция сезонного гриппа сопровождается сменой доминирующего подтипа вследствие антигенной изменчивости и уровня популяционного иммунитета. Мутации в поверхностных белках, особенно в рецептор-связывающем сайте гемагглютинина, позволяют вирусу ускользать от иммунного ответа. Одной из таких мутаций является D222G/N, ассоциированная с повышенной вирулентностью вирусов A(H1N1)pdm09.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: генетическая характеристика вирусов, выявленных в образцах от больных гриппом в Российской Федерации в эпидемическом сезоне 2024–2025 гг.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. С 10 сентября 2024 г. по 19 июня 2025 г. методом ПЦР был исследован 1931 образец от пациентов с диагнозом «грипп», включая 240 образцов аутопсийного материала. Для 420 образцов с достаточным количеством генетического материала получены последовательности генома вируса гриппа с помощью NGS.</p></sec><sec><title>Результаты</title><p>Результаты. В эпидемическом сезоне 2024–2025 гг. в Российской Федерации наблюдалось доминирование вирусов A(H1N1)pdm09. В меньшей мере выявлялись вирусы гриппа B/Victoria, спорадически регистрировались случаи заболевания гриппом A(H3N2). Большинство случаев заболевания гриппом с летальным исходом было связано с группой риска, при этом наблюдался низкий процент вакцинированных лиц. В 19,0 % случаев от исследованных летальных случаев гриппа A(H1N1)pdm09 в мажорном варианте выявлен маркер повышенной вирулентности НА-D222N. В 10,1 % случаях зарегистрировано наличие мутаций D222G/N в минорном варианте. В одном случае заболевания гриппом A(H1N1)pdm09 с выздоровлением (0,5 %) выявлена мутация D222N в мажорном варианте. В шести вирусах от всех исследованных случаев A(H1N1)pdm09 (2,3 %) были выявлены маркеры устойчивости к противовирусным препаратам – осельтамивиру (1,9 %) и балоксавиру марбоксилу (0,4 %).</p></sec><sec><title>Выводы</title><p>Выводы. Выявление мутаций HA-D222G/N, ассоциированных с повышенной вирулентностью и летальностью вирусов A(H1N1)pdm09, подчеркивает важность вакцинации – ключевой меры профилактики, особенно для групп риска, – а также своевременного лечения гриппа.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction: Seasonal influenza circulates in an epidemic pattern, with periodic shifts in the dominant subtype driven by antigenic drift and population immunity. Mutations in surface proteins – particularly within the hemagglutinin receptor-binding site (RBS) – enable the virus to evade host immune responses. Among these, the D222G/N substitution has been associated with increased virulence of A(H1N1)pdm09 viruses.</p></sec><sec><title>Objective</title><p>Objective: Genetic characterization of viruses detected in influenza cases in the Russian Federation in the 2024–2025 epidemic season.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: From September 10, 2024, to June 19, 2025, 1,931 specimens from patients with influenza, including 240 autopsy samples, were tested by RT-PCR. For 420 samples with sufficient genetic material, influenza virus genome sequences were obtained using NGS.</p></sec><sec><title>Results</title><p>Results: During the 2024–2025 flu season, A(H1N1)pdm09 viruses predominated in the Russian Federation. Influenza B/Victoria lineage viruses were less frequent while influenza A (H3N2) cases were sporadic. Most fatal cases occurred in individuals belonging to risk groups, with a notably low vaccination coverage among them. Among fatal A(H1N1)pdm09 cases, the HA-D222N mutation – a marker of enhanced virulence – was detected in the major viral variant in 19.0 % of sequenced samples. Additionally, D222G/N mutations were identified in minor variants in 10.1 % of fatal cases. Notably, the D222N mutation in the major variant was also found in one recovered A(H1N1)pdm09 case (0.5 %).
