Сахарный диабет 2-го типа (СД2) является насущной проблемой глобального здравоохранения, отчасти обусловленной современным образом жизни и влиянием технологий. Циркадные ритмы – биологические часы, регулируемые такими факторами, как свет, сон, гормоны и метаболизм, – играют решающую роль в поддержании гомеостаза глюкозы и координации времени приема пищи. Их нарушение способствует развитию метаболических расстройств, таких как сердечно-сосудистые заболевания, неврологическая дисфункция, иммунная дисрегуляция, рак и плохое качество сна.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: изучить потенциал циркадных методов лечения, как поведенческих, так и фармакологических, в качестве новых стратегий управления СД2.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Был проведен всесторонний обзор литературы с использованием баз данных PubMed, Google Scholar, Web of Science, Scopus и Cochrane, в который были включены только статьи на английском языке, недавно опубликованные в рецензируемых журналах и описывающие исследования, посвященные изучению влияния немедикаментозных подходов, включающих в себя физические упражнения, интервальное голодание и светотерапию, и фармакологических препаратов (например, мелатонина, берберина, орексина и соединений, воздействующих на циркадные ритмы, таких как агонисты REV-ERB и CRY).</p></sec><sec><title>Результаты</title><p>Результаты. Показано, что нарушение циркадных ритмов ухудшает чувствительность к инсулину и регуляцию глюкозы. Вмешательства, восстанавливающие циркадную синхронизацию, такие как светотерапия и суточное ограничение времени приема пищи, улучшают показатели метаболизма. Некоторые препараты, включая мелатонин и берберин, также обладают потенциалом за счет модуляции активности генов биологических часов и улучшения контроля гликемии.</p></sec><sec><title>Заключение</title><p>Заключение. Воздействие на циркадную систему посредством изменения образа жизни и хронофармакологии предлагает многообещающий подход к лечению сахарного диабета 2-го типа. Необходимы дальнейшие исследования для уточнения механизмов, персонализации лечения на основе индивидуальных циркадных ритмов и оценки долгосрочной эффективности.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: Type 2 diabetes mellitus (T2DM) is a growing global health concern, driven in part by modern lifestyle changes and technological influences. The circadian rhythm–a biological clock regulated by factors such as light, sleep, hormones, and metabolism–plays a crucial role in maintaining glucose homeostasis and coordinating meal timing. When disrupted, it contributes to the development of metabolic disorders, including cardiovascular disease, neurological dysfunction, immune dysregulation, cancer, and poor sleep quality.</p></sec><sec><title>Objective</title><p>Objective: To explore the potential of circadian-based therapies–both lifestyle and pharmacological–as emerging treatment strategies for managing T2DM.</p></sec><sec><title> Materials and Methods</title><p> Materials and Methods: A comprehensive literature review was conducted using the PubMed, Google Scholar, Web of Science, Scopus, and Cochrane databases. Only recent English-language peer-reviewed studies on non-pharmacological approaches (e.g., exercise, time-restricted eating, light therapy) and pharmacological agents (e.g., melatonin, berberine, orexin modulators, and clock-targeting compounds like REV-ERB and CRY agonists) were included.</p></sec><sec><title>Results</title><p>Results: Circadian disruption has been shown to impair insulin sensitivity and glucose regulation. Interventions that restore circadian alignment–such as light therapy and meal timing–improve metabolic outcomes. Certain drugs, including melatonin and berberine, also exhibit potential by modulating clock gene activity and enhancing glycemic control.</p></sec><sec><title>Conclusion</title><p>Conclusion: Targeting the circadian system through lifestyle adjustments and chronopharmacology offers a promising approach to managing T2DM.
