В соответствии с основополагающими принципами международных рекомендаций об «особом значении наличия квалифицированных мотивированных медицинских кадров, располагающих необходимой поддержкой» (Декларация ВОЗ, 2019), оценка факторов, влияющих на удовлетворенность работой врачей общей практики (ВОП), относится к области прикладных исследований качества медицинской помощи и достижения удовлетворенности пациента.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: развитие методологии и технологических инструментов измерения и оценки факторов производственной среды, оказывающих влияние на удовлетворенность работой ВОП.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Применены методы анкетирования, статистического анализа и обобщения ответов 340 ВОП (СВ) амбулаторно-поликлинического звена учреждений здравоохранения г. Москвы с использованием электронной русскоязычной версии международного опросника COPSOQ III (Long version) (2022 г.). Настоящее исследование выполнено в рамках расширенного обобщения полученных данных о влиянии факторов производственной среды на психосоциальное состояние врачей общей практики в ПМСП и обоснования методологических подходов к оценке их удовлетворенности работой. Дополнительно на основе нормализации показателей и ранжирования факторов психосоциальных рисков обосновано применение методологического подхода к статистической обработке данных. Уровень статистической значимости был зафиксирован на уровне р ≤ 0,05.</p></sec><sec><title>Результаты исследования</title><p>Результаты исследования. На основе результатов количественного анализа нормализованных средних оценок психосоциальных факторов производственной среды выявлены достоверные различия в группах «низкий», «норма» и «высокий» уровней удовлетворенности работой ВОП (р &lt; 0,0001), Определен перечень из 12 факторов, определяющих высокий уровень неудовлетворенности ВОП работой.</p></sec><sec><title>Ограничения исследования</title><p>Ограничения исследования: опрос проводился среди врачей общей практики ПМСП в г. Москве.</p></sec><sec><title>Заключение</title><p>Заключение. На основе применения дополнительных статистических методов нормализации средних значений оценочных результатов опроса и их ранжирования по степени значимости предложен методологический подход к анализу факторов риска производственной среды, позволяющий получить достоверные уровни по показателю удовлетворенности работой ВОП и сравнимые с данными литературы. Апробированная методология рекомендована для контроля качества медицинской помощи в здравоохранении.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction: In line with the fundamental principles of the international guidelines on particular importance of “having a competent, motivated and supported health workforce” (WHO Declaration, 2019), the assessment of factors affecting job satisfaction of general practitioners (GPs) falls within the field of applied research on the quality of health care and achievement of patient satisfaction.</p></sec><sec><title>Objective</title><p>Objective: To develop the methodology and techniques for measuring and evaluating work environment factors affecting job satisfaction of GPs.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: We applied methods of questioning, statistical analysis, and summarizing of responses from 340 GPs of Moscow outpatient health facilities using the electronic Russian-language version of the international COPSOQ III questionnaire (Long version; 2022). The study was carried out as part of an expanded generalization of the data on the influence of workplace factors on the psychosocial well-being of general practitioners in outpatient clinics and substantiation of methodological approaches to assessing their job satisfaction. Additionally, based on the normalization of indicators and the ranking of psychosocial risk factors, we provided a rationale for the application of a methodological approach to statistical data analysis. The significance level was set to 5 % (i.e. p ≤ 0.05).</p></sec><sec><title>Results</title><p>Results: Based on the results of a quantitative analysis of normalized mean estimates of psychosocial factors of the work environment, we revealed significant differences between the groups of “low, “normal” and “high” levels of GPs’ job satisfaction (p &lt; 0.0001) and compiled a list of 12 factors determining a high level of job dissatisfaction.</p></sec><sec><title>Study limitation</title><p>Study limitation: The survey was conducted among the general practitioners of outpatient departments of Moscow hospitals.</p></sec><sec><title>Conclusions</title><p>Conclusions: Based on the use of additional statistical methods for normalizing the mean values of the estimated survey results and ranking them by degree of significance, a methodological approach to the analysis of risk factors of the occupational environment in medical organizations is proposed that helps obtain reliable levels in terms of job satisfaction compliant with literature data.
