Одной из серьезных проблем здравоохранения являются острые кишечные инфекции, вызываемые преимущественно вирусами, большинство из которых распространяются через загрязненную фекалиями окружающую среду. В Дальневосточном федеральном округе Российской Федерации случаи кишечных инфекций ежегодно регистрируются на высоком уровне. При этом более 50 % заболеваний установленной этиологии обусловлены ротаи норовирусами. Кроме того, практически во всех субъектах Дальневосточного федерального округа, в том числе и в Хабаровском крае, ежегодно регистрируются очаги групповой заболеваемости, вызванные кишечными вирусами.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: провести анализ заболеваемости острыми вирусными кишечными инфекциями в Хабаровском крае в 2022 году, проанализировать результаты молекулярно-генетического мониторинга кишечных вирусов, обусловивших групповую заболеваемость в крае в 2022 году.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Использованы эпидемиологический, статистический методы анализа заболеваемости. Молекулярно-генетическими методами исследованы пробы клинического материала от больных острыми кишечными инфекциями, контактных с больными, собранные в очагах кишечных инфекций в Хабаровском крае.</p></sec><sec><title>Результаты</title><p>Результаты. Эпидемиологическая ситуация по острым кишечным инфекциям в Хабаровском крае в 2022 году в целом была неблагополучной. Показатель заболеваемости острыми кишечными инфекциями превысил на 20,6 % уровень по округу, при этом среди острых кишечных инфекций установленной этиологии в большинстве случаев отмечались инфекции, вызванные кишечными вирусами, а также зарегистрированы множественные очаги групповой заболеваемости. Эпидемиологическое расследование 6 очагов кишечных инфекций установило фекально-оральный механизм передачи инфекции, реализованный пищевым, контактно-бытовым и, вероятно, водным путями. Проведенные молекулярно-генетические исследования кишечных вирусов, вызвавших вспышки острых кишечных инфекций в Хабаровском крае, идентифицировали такие генотипы норовирусов, как GII.4Sydney[P16], GII.4Sydney[P31], GII.17[P17] и GII.6[P7], а также ротавирусов А – G9P[<xref ref-type="bibr" rid="cit8">8</xref>], G4P[<xref ref-type="bibr" rid="cit8">8</xref>] и G3P[<xref ref-type="bibr" rid="cit8">8</xref>]. Установлено, что некоторые из обнаруженных генотипов норовирусов ранее вызывали вспышки групповой заболеваемости в Хабаровском крае: GII.17[P17] – в 2015 году среди населения поселка Многовершинный, GII.6[P7] – в 2018 и 2019 году в г. Хабаровске, что указывает на длительную циркуляцию данных генотипов.</p></sec><sec><title>Заключение</title><p>Заключение. Для изучения вклада в заболеваемость острыми кишечными инфекциями разных геновариантов рота- и норовирусов необходимо отслеживать изменения типовой структуры вирусной популяции. В связи с этим целесообразно проводить молекулярное типирование рота- и норовирусов не только при расследовании очагов групповой заболеваемости, но и при регистрации спорадической заболеваемости острыми кишечными инфекциями.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction: Acute intestinal infections are amongst the most important health concerns worldwide. The majority of them are caused by viruses spread through environmental objects contaminated with feces. A high incidence of intestinal infections is registered annually in the Far Eastern Federal District of the Russian Federation. Over 50 % of cases of established etiology are induced by rota- and noroviruses. Moreover, outbreaks of viral intestinal infections are registered annually in almost all regions of the Far Eastern Federal District, including the Khabarovsk Krai.</p></sec><sec><title>Objective</title><p>Objective: To analyze the incidence of acute intestinal infections and the results of molecular genetics testing of intestinal viruses that caused the outbreaks in the Khabarovsk Krai in the year 2022.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: We used epidemiological and statistical methods to analyze the incidence and conducted molecular genetics testing of samples obtained from patients with acute intestinal infections and their contacts in the foci of the disease in the Khabarovsk Krai.</p></sec><sec><title>Results</title><p>Results: The acute intestinal infection situation in the Khabarovsk Krai in 2022 was unfavorable. The regional incidence rate was 20.6 % higher than that in the Far Eastern Federal District, with most diseases induced by intestinal viruses and numerous outbreaks registered. Our epidemiological investigation of six outbreaks revealed the fecal-oral transmission route of infection through contaminated food, household contacts and, probably, water. Molecular genetics testing of the causes of outbreaks showed circulation of norovirus genotypes GII.4Sydney[P16], GII.4Sydney[P31], GII.17[P17], and GII.6[P7] as well as rotavirus A genotypes G9P[<xref ref-type="bibr" rid="cit8">8</xref>], G4P[<xref ref-type="bibr" rid="cit8">8</xref>] and G3P[<xref ref-type="bibr" rid="cit8">8</xref>] in the Khabarovsk Krai. Some of the identified norovirus genotypes had already caused outbreaks in the Khabarovsk Krai: GII.17[P17] – back in 2015 among the population of the Mnogovershinny village, GII.6[P7] – in the years 2018 and 2019 in the city of Khabarovsk, thus indicating long-term circulation of these genotypes.</p></sec><sec><title>Conclusion</title><p>Conclusion: Surveillance over changes in genetic diversity of the viral population is required to evaluate contribution of various gene variants of rota- and noroviruses to the incidence of acute intestinal infections.
