Известно, что фекальный кальпротектин является биомаркером воспалительного повреждения кишечника. Повышение его уровня отмечается также при инфекции COVID-19, что связано с патогенным действием вируса на эпителий кишечника с привлечением компонентов неспецифического звена иммунитета.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: оценить маркеры местного и системного воспаления и их взаимосвязь при диарейном синдроме при новой коронавирусной инфекции COVID-19.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проанализированы данные 90 историй болезни пациентов в возрасте от 35 до 70 лет с лабораторно верифицированной SARS-CoV-2-инфекцией. Пациенты (n = 90) были разделены на три группы в зависимости от уровня фекального кальпротектина: первая группа соответствовала уровню фекального кальпротектина более 200 мкг/г (n = 30), вторая – от 50 до 200 мкг/г (n = 30) и третья – до 50 мкг/г (n = 30).</p></sec><sec><title>Результаты</title><p>Результаты. У пациентов с уровнем кальпротектина более 200 мкг/г в 1,5 раза чаще встречалось тяжелое течение заболевания, объем поражения легких составил более 50 % (p &lt; 0,05), продолжительность лихорадки и диареи сохранялась до 14,8 ± 0,5 (p &lt; 0,05) и 8,7 ± 0,4 дня (p &lt; 0,05) соответственно.</p></sec><sec><title>Заключение</title><p>Заключение. Установлена прямая взаимосвязь маркеров местного и системного воспаления. Клинические проявления новой коронавирусной инфекции COVID-19 коррелируют с выраженностью интестинального воспаления в кишечнике. Выявлена положительная корреляционная связь между уровнями фекального кальпротектина и длительностью диарейного синдрома. Пейзаж условно-патогенной микробиоты кишечника коррелирует с выраженностью воспалительного процесса в кишечнике.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction: Fecal calprotectin is known to be a biomarker of inflammatory bowel problems. An increase in its level has been also noted in COVID-19 patients, which is associated with the pathogenic effect of the virus on the intestinal epithelium involving components of nonspecific immunity.</p></sec><sec><title>Objective</title><p>Objective: To evaluate markers of local and systemic inflammation and their relationship in the diarrhea syndrome associated with the novel coronavirus disease (COVID-19).</p></sec><sec><title>Materials and methods</title><p>Materials and methods: We have analyzed medical histories of 90 laboratory-verified SARS-CoV-2 patients aged 35 to 70 years and divided them into three equal groups of 30 cases each by the measured level of fecal calprotectin (Group 1: CPT &gt; 200 μg/g, Group 2: CPT = 50 to 200 μg/g, and Group 3: CPT &lt; 50 μg/g).</p></sec><sec><title>Results</title><p>Results: In Group 1 patients with CPT &gt; 200 μg/g, we observed a 1.5 times more frequent severe course of the coronavirus disease, pulmonary damage over 50 % (p &lt; 0.05), fever and diarrhea that persisted up to 14.8 ± 0.5 and 8.7 ± 0.4 days, respectively (p &lt; 0.05).</p></sec><sec><title>Conclusions</title><p>Conclusions: We established a direct relationship between markers of local and systemic inflammation. Clinical manifestations of COVID-19 correlated with the severity of intestinal inflammation. A positive correlation was found between fecal calprotectin levels and diarrhea syndrome duration.
